It is best to prepare for your visit by downloading and filling out the Health History and Consent to Treat forms. Before your visit please download the Health History Form, fill it out and bring it you to your appointment.

We also recommend that you download the Treatment Process and Preparation file and read it over before your visit. This will help orient you to the treatment process, and give you an idea of what you can expect from your treatment at Yong Kang Clinic. This is especially helpful if you have not had acupuncture, or not received acupuncture from someone trained in Chinese medicine.

Download them as well. Read through the consent form. If you have questions, we will address those as our first order of business. If not, please sign the form and bring it with you.

Appointments are reserved exclusively for you. A minimum of 48 hours’ notice is required for cancellations or changes. Late cancellations will incur a fee.

Additional time is reserved for all new patient visits to allow for a comprehensive health history and treatment planning. Because of the limited availability of these longer sessions, new patient appointments that are canceled without adequate notice or missed without notice are not able to be rescheduled.
